چکیده

Fatal air embolism to the cerebrum during an endoscopic retrograde cholangiopancreatography (ERCP) has not been reported in a patient with a biliodigestive anastomosis and multiresistant extended-spectrum β-lactamase Escherichia coli (ESBL) bacteremia. A 59-year-old woman with a history of laparoscopic cholecystectomy and iatrogenic injury of the right choledochal duct, choledochojejunostomy (biliodigestive anastomosis), recurrent cholangitis, revision of the biliodigestive anastomosis, recurrent liver abscesses, and recurrent stenting of stenotic bile ducts, was admitted because of fever and tenderness of the right upper quadrant. On ERCP, a previously deployed covered Wallstent was replaced. Blood cultures grew ESBL. After stent removal 8 days later, the patient did not wake up and developed arterial hypotension and respiratory insufficiency, requiring mechanical ventilation. Computed tomography scans showed extensive air embolism to the liver, heart, and cerebrum. She died 1 day later. Although the exact pathogenesis of the fatal cerebral air embolism remains speculative, the nonphysiological anatomy and chronic infection with ESBL may have been contributory factors.

A Cerebral Air Embolism after Endoscopic Treatment

A 77-year-old man with liver cirrhosis was admitted to our hospital in 2015. He was unconscious and his Glasgow Coma Scale (GCS) score was 10. The Hb level was 3.5 mg/ dL. Contrast CT showed esophageal varices and atrophy of liver, these findings indicated a suspicion of liver cirrhosis. We considered that the varix rupture and the resulting hemorrhagic shock were the main cause of the patient’...

Fatal Cerebral Air Embolism: A Case Series and Literature Review

Cerebral air embolism (CAE) is an infrequently reported complication of routine medical procedures. We present two cases of CAE. The first patient was a 55-year-old male presenting with vomiting and loss of consciousness one day after his hemodialysis session. Physical exam was significant for hypotension and hypoxia with no focal neurologic deficits. Computed tomography (CT) scan of head showe...

Patent Foramen Ovale Closed Before Robotic Radical Prostatectomy

Introduction: Robotic prostatectomy is considered by many urologists as the new standard of care. Cases Description: We encountered 2 patients scheduled for a robotic prostatectomy whose past medical history was remarkable for a cerebral palsy that had been considered to be due to a patent foramen ovale. At this time, it was decided that foramen ovale closure was not necessary. Because of the h...

Life-threatening air embolism during ERCP.

Patient #1, a 45-year-old man who was being monitored because of chronic pancreatitis, had anicteric cholestasis and pain caused by double-duct stricture. Endoscopic retrograde cholangiopancreatography (ERCP) was undertaken using air, with the patient under general anesthesia with intubation (●" Fig.1). After short sphincterotomies had been done, self-expandingmetal stents were inserted (10mm d...

Fatal cerebral fat embolism associated with a patent foramen ovale.

A healthy 22-year-old woman developed respiratory failure and brain death within 48 h of bilateral tibial fractures. Autopsy showed fat embolism in the lung and brain, and a large patent foramen ovale that may have contributed to massive cerebral fat embolism.
